Persons Insured by this Policy:

The policy provides cover for the following:

  • All full time/part time students of UWA
  • Active life and active associate members of UWA Guild of Undergraduates UWA Sports and affiliated and or related entities
  • Members (Active/Associate/Life) of registered sporting clubs of UWA, UWA Guild & UWA Sports Association 

Benefits:

Accidental Death $10,000 for persons without dependants $100, 000 for persons with dependants
Permanent Disability$100,000
Weekly Benefits (Income Earners)$250 per week, 104 week Benefit
Domestic Help (non-income earners)$250 per week, 104 week Benefit
Surgical Benefits$2,000 maximum
Non Medicare Medical Expenses80% of costs to a $5,000 maximum
Student Contribution and/or Post Graduate Fees$10,000 maximum reimbursement following Total Disablement
Home Tutorial Expenses85% of costs with a 7 day excess, $250 per week, 52 week

Conditions:

Medical expenses which can be claimed either in part or in full through Medicare cannot be claimed on this policy

Excess:

The policy is subject to a $50 excess eg if your claim was for $150 and the claim is accepted you will receive $100.

Making a Claim: 

Claims must first be lodged with your private health insurer or relevant sports association.

Download a claim form and Attending Physicians Statement form, complete and return them to the Insurance Officer with all receipts, quotes and relevant documentation. 

Please do not submit accounts, they must be paid for by the student in the first instance and a reimbursement claimed through insurance.

Once your claim has been submitted to Insurance (M451)

  • Your claim will be submitted to the insurers who have 14 days to assess your claim. They will request any further information they require during this period
  • If your claim is accepted, a cheque will be issued to UWA. The cheque will be banked and a cheque will be drawn in your name. This cheque will be forwarded to you as soon as possible
