Persons Insured by this Policy 

The policy is available to all staff holding a 457 visa who have been appointed from overseas on a short term contract where the University is required to sponsor the employee. 

Cover is also available for partners and children.

This cover only applies to treatment received in Australia.
 

Benefits

The policy provides full medical cover and limited cover for optical and dental treatment received in Australia.

The Policy Does Not Cover

  • SERVICES RECEIVED OUTSIDE OF AUSTRALIA.
  • Cosmetic, elective or plastic surgery, (except and to the extent that it is necessary for the cure or alleviation or Injury to the Insured Person) or out of any routine physical examinations.
  • Pre-existing Medical Conditions are subject to a three month period waiting period prior to commencement of cover.
  • Treatment or services which are covered by Medicare or by compensation under any Workers Compensation Act, or Transport Accident Laws or by any Government sponsored Fund, Plan, or Medical Benefit Scheme or under any other medical insurance policy.
  • Charges for non-medical services including but not limited to telephone, television, newspapers and the like.
  • Acquired Immune Deficiency Syndrome (AIDS) or Human Immunodeficiency Syndrome (HIV) infection however caused.
  • Sexually transmitted disease, infertility, sterilisation, suicide or any attempt thereat, intentionally self-inflicted injuries, abortion (unless certified as medically necessary by the attending Doctor or Specialist), congenital deformities or abnormalities.
  • Any expense or charges incurred after a person has ceased to be an Insured Person, or Spouse, or Dependent Child under this policy,  or after this Policy has been terminated or after the date the Insured Person attains the age of 65 years.
  • The use of alcohol or intoxicants, or the use of drugs except as prescribed by a Doctor or Specialist.
  • Any consequence of war, invasion or civil war.
  • Radioactive contamination or radioactivity in any form whatsoever whether occurring naturally or otherwise.
  • Any expenses or changes incurred after the expiry date of the Policy Period.

Premium

The premium on the policy can either be paid by fortnightly payroll deductions or by a lump sum payment in advance. The rates for 2007/8 are as follows:

Single    $972 per annum
Couples  $1,950 per annum
Family    $2,925 per annum

Excesses/Deductibles 

The policy is subject to a $100 excess per year.

Making a Claim 

Pay the accounts incurred and submit the ORIGINAL receipts along with a claim form to Insurance at M451.    

Please note:  Photocopies of receipts and or accounts will no longer be accepted.          

Claim Form


 Once your claim has been submitted to Insurance (M451)

  • Your claim will be submitted to the insurers who have 14 days to assess your claim.  They will request any further information they require during this period.
  • If your claim is accepted, a cheque will be issued to UWA.  The cheque will be banked and a cheque drawn in your name.  This cheque will be forwarded to you as soon as possible.
  • If the cheque is under $100, you may be asked to collect as cash from the Cashier.
  • Please also be aware that this policy has a $100 per policy excess applied.  If you have any queries, please forward to insurance@uwa.edu.au.
